29 CFR 1926.554(a)(2): Supporting structure(s) to which overhead hoists were attached did not have a safe working load equal to that of the hoist: The employer is failing to protect workers from struck-by hazards. Workers of a HVAC subcontractor were walking down a set of stairs on site and suffered injuries during the collapse of the stairs. The iron working subcontractor installing the set of steel stairs on the floor below used the set of wooden stairs above them to hoist the steel pan stairs into place. The iron workers failed to ensure that the wooden stairs met or exceeded the load capacity of the hoist. The HVAC workers walked on the wooden stairs as the iron workers were lifting the steel pan stairs into place and as they were walking on the stairs the wooden stairs gave way and the workers fell approximately 15 feet to the concrete floor of the stairwell.

29 CFR 1926.1051(a)(3): A second point of access was not provided and used where a building or structure had only one point of access between levels and free passage at that point of access was restricted as a result of work that must be performed: The employer is failing to protect employees from fall hazards by not ensuring that two points of access and egress are provided for workers on site. Employees of the HVAC subcontractor suffered injuries after they were forced to use a stairwell which was being worked on. The employees went down the stairs at the same time the ironworker's were installing steel pan stairs below them. As they walked on the stairs above the installation, the stairs collapsed causing the employees to fall approximately 15 feet to the concrete floor of the stairwell. The ironworker's had installed another set of stairs earlier in the day on the East side of the site and this stairwell was barricaded. As they moved to the West side stairs no one barricaded the West stairs and left the East side barricaded forcing workers to walk down the stairs that were being installed.
