29 CFR 1910.266(i)(3): The training provided by the employer did not include the elements specified in 1910.266(i)(3)(i) through (vi):    The employer does not provide employees with training in accordance with the requirements of the logging standard include the elements specified in 1910.266(i)(3)(i) through (vi). This violation occurred on or about January 22, 2014, and at times prior thereto, at the working site when employees were required to perform logging work without being trained in accordance with the requirements of the logging standard include the elements specified in 1910.266(i)(3)(i) through (vi).         Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all employees are trained in accordance with the requirements of the logging standard including the elements specified in 1910.266(i)(3)(i) through (vi).

29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident.  The employer does not report the death of employees when the death is work-related. This violation occurred on or about January 22, 2014, when the employer failed to orally report to OSHA the death of an employee within eight hours.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all work-related fatalities and in-patient hospitalizations of three or more employees are reported orally to OSHA within 8 hours of the event.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met.     The employer has not developed or implemented a written hazard communication program that describes how they comply with this standard regarding labels and other forms of warning, material safety data sheets, and employee information and training. This violation occurred on or about January 22, 2014, and at times prior thereto, when employees performing logging work were required to handle hazardous chemicals before a written hazard communication program had been developed and implemented.          Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a written hazard communication program is developed, implemented, and maintained that includes the criteria specified in 29 CFR 1910.1200(f-h).
