29 CFR 1910.23(a)(8)(ii)i: Every floor hole into which persons can accidentally walk was not guarded by by a floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing: a) Wash Room Behind Washer #4 - A trough cover had slid down and created a 5 inch by 11 inch opening which was 3 inches to 10 inches deep, on or about February 5, 2014. b) Wash Room Behind Washer #4 - A 10.25 inch by 22 inch trench that was carrying piping and was 3.5 inches deep was unprotected, on or about February 5, 2014. c) Wash Room Behind Washer #1 - A trough cover had slid down and created a 4.5 inch by 8 inch opening which was 10 inches deep, on or about February 5, 2014.
