29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter: At the inspection site, employees who were required to wear tight-fitting half-facepiece respirators with P100 filters for protection from respirable crystalline silica while conducting cleaning operations were not provided annual fit testing. The Halliburton Energy Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2) which was contained in OSHA inspection number 315558684, citation number 1, item number 1, and was affirmed as a final order on 08/08/2012 with respect to a workplace located at 101 Lucerne Road, Homer City, PA 15748.

29 CFR 1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked in accordance with paragraph (f)(6)(i) or (f)(6)(ii) of this section: a) At the inspection site, employees were filling fuel tanks on track mobiles with combustible diesel fuel from a Startex Tank Rentals storage tank, and the tank was not labeled with the contents or hazard warnings. b) In the storage building, a gallon jug containing transmission fluid used by employees to top off the reservoir for the boot seal was not labeled with the contents or hazard warnings.
