29 CFR 1910.132(e): Defective or damaged personal protective equipment continued to be used: On or about January 30, 2014 the employer had not maintained the face shield in proper condition that was available and in use in the laundry area when the barrels of corrosive chemicals are handled, changed out, etc. The face shield was observed to be broken into two parts.

29 CFR 1910.151(c): Where eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drench or flushing of the eyes and body were not provided with the work area for immediate emergency use: On or about January 30, 2014, the employer had not provided adequate, suitable eye wash/quick drench facilities, with a minimum 15 minutes of flushing capability, in the laundry facility at or near where corrosive chemicals such as, but not limited to bleach, detergents, and fabric softners are distributed/dispensed to the laundry processing equipment.
