29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  On January 29, 2014 at the jobsite located at or near 5304 Municipal Ave, in Kansas City, MO, employees were exposed to a struck by hazard in that the employee was working in area where there was a possible head injury from impact and was not protected by protective helmet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On January 29, 2014 at the jobsite located at or near 5304 Municipal Ave, in Kansas City, MO employees were exposed to a fall hazard in that they were working from a surface approximately 20 feet above the next level without an approved form of fall protection.
