29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about February 12, 2014, the employer, Rocky Mountain Marble, LLC, did not ensure employees, who were required to engage in the production of cultured marble products, were provided adequate personal protective equipment so as to protect them from skin contact to methyl ethyl ketone peroxide and styrene polymer.
