29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to the items of this section: 237 N. 13th St., Blair, NE 68008 (Site-Wide): The employer did not ensure that the lockout procedures used by employees to service equipment were adequately specific. The procedures had partially generic (not specific) steps for shutting the equipment down, and did not have specific procedures for starting the equipment up, including, but not limited to, the Hydraulic Shear #1802. Abatement certification and abatement documentation is required for this violation. The abatement certification sheet is enclosed.

29 CFR 1910.178(m)(5)(i): Powered industrial trucks were left unattended without the power being shut off: 237 N. 13th St., Blair, NE 68008 (East Warehouse Yard): The employer is failing to ensure that employees who were allowed to operate forklifts shut the power off on the forklift running while unattended. A Hyster Forklift was left running while the forklift operator was inside a building approximately 23 feet away from the door with the door closed. Abatement certification is required for this violation. Abatement certification sheet is enclosed with the citation.
