29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a. On or about January 30, 2014, at the above addressed jobsite, sixteen employees were exposed to amputation hazards while performing maintenance and cleaning of thirty seven "Injection Molding" machines, for which the Energy Control Procedures did not clearly and specifically outline, the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. On or about January 30, 2014, at the above addressed jobsite, employees were operating a rivet machine, to install metal rivets into plastic components, which was not guarded at point of operation.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a. On or about January 30, 2014, at the above addressed jobsite, two employees were operating a pedestal mounted abrasive wheel grinder, to fabricate various metal parts, for which the work rest(s) were greater than 1/8 inch from the grinding wheel.
