OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards associated with improper automobile lift operations:    (a) On or about January 29, 2014, and at times prior, Pep Boys employees were exposed to crushing and struck-by hazards while working around automotive lifts as follows:    -Lifts were not inspected daily during use to ensure that they were properly maintained and functional.  -Safety locks on in-ground lift #1 were not freely engaging in the fully extended position.      Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    Pep Boys was previously cited for a violation of this occupational safety and health standard or its equivalent standard , which was contained in OSHA inspection number 923530, citation number 1, item number 1 and was affirmed as a final order on August 26, 2013 , with respect to a workplace located at 12820 E. Mississippi Avenue, Aurora, CO 80012.    Pep Boys was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 914663, citation number 1, item number 1 and was affirmed as a final order on August 1, 2013, with respect to a workplace located at 5134 S. Wadsworth Blvd. Lakewood, CO 80123.
