29 CFR 1910.176(a): Permanent aisles or passageways were not appropriately marked: (a) On, and at certain times before, January 24th, 2014, employees in the warehouse area of the plant were exposed to possibly being struck by materials handling equipment in that permanent aisleways had not been marked to ensure safe equipment operation and pedestrian safety. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(3)(ii): The points of operation of machines whose operations exposed employees to injury were not guarded: (a) On, and at certain times before, January 24th, 2014, employees in the Molding Maintenance area of the plant were exposed to cut, laceration and possible amputation hazards while operating vertical bandsaws in that blade areas above the adjustable blade guides were unguarded. These instances included, but were not necessarily limited to, the following exposures: (1) The Roll-In Saw, #EF1459, vertical bandsaw had 8 inches of exposed blade above the adjustable blade guide, and (2) The Rockwell Delta 20-inch bandsaw had 5 inches of similarly exposed blade. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
