On or about January 31, 2014, an employee working from the 2nd floor balcony was not protected from the hazard of falling 10 feet by the use of guardrails, safety nets, or personal fall arrest systems. b. On or about January 31, 2014, an employee working from the 3rd floor balcony was not protected from the hazard of falling 20 feet by the use of guardrails, safety nets, or personal fall arrest systems.
