29 CFR 1910.1200(h)(2)(iii): The location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets required by this section. Warehouse/Checker Station/Loading Trucks Operations - On or about January 31, 2014, the employer had not provided information and training to (all) employees from second shift on the availability of the hazard communication program and all its elements including the list of hazardous chemicals and the safety data sheets (propane - C3H8). The employees missing information on the hazard communication included: 1. No certification of information and training conducted for seven (7) employees with the title as "checkers". 2. No certification of information and training conducted for seven (7) employees with the title as "supervisors.

29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable. a. Warehouse/Southside Section - On or about January 31, 2014 the employer did not ensure the working floors were maintained clean, dry, and an orderly manner. Employees were thereby exposed to the hazards of slips, trips, and falls.
