29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket while working from an aerial lift. At the worksite: On or about January 30, 2014 the employer allowed an employee to use an aerial lift without securing themselves to the boom or basket. The employee was exposed to fall from height hazards.
