29 CFR 1910.147(a)(3)(i): Employer did not utilize procedures for affixing appropriate lockout devices and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees. a) On or about 2/3/14, employees did not follow written lockout procedures for cleaning the bucket on the Peters machine. Employees were only using the E stop button as lockout procedure. No Abatement Certification Required:
