On or about February 12, 2014, Store Room:  Access to the designated emergency exit was reduced to less than 28 inches by the storage and placement of cardboard boxes and plastic bins.  Clearance was measured at 12 1/2 inches.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A.  On or about February 12, 2014, Bakery:    1.  An Economatic 3-pie shell forming machine was unguarded, exposing employees to crushing injuries from the heated die.    2.  The Colborne Dough Divider, Model 52 was missing a barrier guard on the front of the equipment, exposing employees to crushing injuries from the divider knife.

On or about February 12, 2014,  the following vertical mixers were not provided with enclosure guarding or other means to prevent contact with moving parts:    1.  Bakery, near ovens:  Hobart 5 HP, Model V-1401 vertical mixer.    2.  Bakery, near dough cutter:  Artofex twin-arm vertical mixer.    3.  Bakery, near dough cutter:  F.W. Pagani, Model 11 twin-arm vertical mixer.    4.  Cream Room:  Hobart 2 HP, Model M-802 vertical mixer.
