29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Jobsite: 121 Pembroke Court, Roof - Employees installing roof shingles on a steep roof (more than 4 in 12) were exposed to fall hazards of approximately twenty (20) feet without the use of guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Observed on or about 10/31/13.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.  (a) Jobsite: 121 Pembroke Ct., Roof - The portable extension ladder used to access the roof did not extend at least three (3) feet above the upper landing surface exposing employees to a fall hazard during access and/or egress from the roof.  Employees were installing a new roof approximately twenty (20) feet above the ground.  Observed on or about 10/31/13.
