29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.      a) Jobsite: 100 Block S. Walnut St. - An employee conducting bolting up operations on the beams and columns of the steel structure with unprotected sides more than twenty-two (22) feet above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. Observed on or about 11/21/13.

Walnut St. - An employee conducting bolting up operations on an upper level was exposed to the hazards of impalement by protruding reinforcing steel which was not guarded to eliminate such hazard. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      a) Jobsite: 100 Block S. Walnut St. - The portable extension ladder used to access an upper level did not extend at least three (3) feet above the upper working surface exposing employees to a fall hazard during access and/or egress.  An employee was conducting bolting up operations on the steel structure approximately twenty-two (22) feet above the ground level. Observed on or about 11/21/13.
