29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  a) Jobsite at 212 W Roosevelt, Nampa, Idaho:  The employer, 208 Roofing & Construction, did not provide certification and documentation that SERIOUS citation 1, item 1a of inspection number 957079 were corrected.  The latest date by which this violation was to be corrected was March 4, 2014.  b) Jobsite at 212 W Roosevelt, Nampa, Idaho:  The employer, 208 Roofing & Construction, did not provide certification and documentation that SERIOUS citation 1, item 1b of inspection number 957079 were corrected.  The latest date by which this violation was to be corrected was March 4, 2014.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure.  a) On or about January 31, 2014, 212 W Roosevelt, Nampa, Idaho:  Employees were working on a 10:12 pitch roof without means of fall protection in the workplace.  The potential fall distance employees were exposed to was measured at 10.7 feet at the eave.  Abatement certification is required for this item.

29 CFR 1926.503(b)(1) The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record did not contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   a) On or about January 31, 2014, 212 W Roosevelt, Nampa, Idaho:  Employees were working on a 10:12 pitch roof without means of fall protection in the workplace.  The potential fall distance employees were exposed to was measured at 10.7 feet at the eave.    The employer did not have a written certification of training of employees who were exposed to this fall hazard.  Abatement certification is required for this item.
