29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Excavation Site: On or about January 27, 2014, and at times prior, employees were exposed to a fall of approximately 20 feet to the ground below while accessing and egressing Reactor 3. Employees would have to stand on top of the wall and turn around to begin their decent on the second ladder.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder: (a) Excavation Site: On or about January 27, 2014, and at times prior, employees were exposed to a fall of approximately 20 feet to the ground below while accessing and egressing Reactor 3.
