29 CFR 1926.451(c)(2)(v): The employer used a forklift to support a scaffold platform without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied. On or about 1-23-14, the employer failed to ensure that a scaffold platform being supported by a forklift was attached to the forks. An employee was observed standing on the platform while the forks were raised, lowered and moved horizontally. The platform was raised to approximately 11 feet high.
