29 CFR 1910.23(d)(1)(iv): The employer did not ensure that stairs with four or more risers that were more than 44 inches wide but less than 88 inches wide had a stair railing on each open side and a handrail on each enclosed side.   (a) South Dock, between dock and North end of Recycling Trailer - On or about January 23, 2014, employees were exposed to falls of up to 7 feet due to the absence of stair railings on each open side of the 5 riser-high by 47 3/4 inches-wide wooden stairs.

29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work and/or were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.   (a) Southeast corner of Press Room - On or about January 23, 2014, employees using the unidentified brand, 1/4 horsepower, bench grinder made in 1984, serial number 25639, were exposed to injury due to there being no work rests on either side of the grinder.
