29 CFR 1910.178(g)(11): Precautions were not taken to prevent open flames, sparks, or electric arcs in battery charging area.    On or about 14 January 2014, and times there prior to, the employer did not ensure that precautions were taken to prevent open flames, sparks, or electrical arcs in battery charging area, where electric fans that could create sparks or electric arcs were being used near battery charging areas.    a. Where one fan was mounted on the wall in battery charging room.    b. Where fan was placed infront of intake on back wall of battery charging room.      The United States Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(g)(11), which was contained in OSHA inspection number 315436014, citation number 01, item number 002 and was affirmed as a final order on August 24, 2011, with respect to a workplace located at 7001 S Central Ave, Los Algeles CA, 90052.    The United States Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(11), which was contained in OSHA inspection number 895755, citation number 01, item number 004 and was affirmed as a final order on May 31, 2013, with respect to a workplace located at 7001 S Central Ave, Los Angeles CA 90052.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    On or about 19 September 2013, the employer did not ensure that employees assigned to use powered industrial trucks (PIT), completed the training required under paragraph(l):     a. In where the untrained and untested employee lead to an injury on 19 Sept 2013 where an employee fractured his ankle.    b. In where supervisors allowed additional untrained and untested employees to operate powered industrial vehicles without having the appropriate prior training for the equipment assigned during the days of September 14 through September 19 2013.      The United States Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(ii), which was contained in OSHA inspection number 389742, citation number 01, item number 01, and was affirmed as a final order on September 30, 2013, with respect to a workplace located at 1501 S Cherrybell Stravenue, Tucson, AZ 85726.

29 CFR 1910.178(q)(7): Defects when found were not immediately corrected.    On or about 14 January 2014, the employer did not ensure that defective batteries were immediately taken out of service:    a. Where battery number 5 had damaged wires and it was connected to 24 V charger number 2.    b. Where battery number 28 had damaged wires on a motorized pallet jack number 37 .    c. Where a battery had a damaged wire that was tapped on motorized pallet jack (number 34) in use by employee.    The United States Postal Service was previously cited for a REPEAT violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(7), which was contained in OSHA inspection number 928063, citation number 02, item number 001 and was affirmed as a final order on November 13, 2013, with respect to a workplace located at 5555 Bandini Blvd, Bell CA 90201.
