29 CFR 1910.179(n)(3)(v): While any employee is on the load or hook, the employer did not ensure there was no hoisting, lowering, or traveling:    a) Job site, 4990 Grandview Avenue, Neville, Island, PA: On or about January 27, 2014, during production operations at Bay number 1, the employer did not ensure loads were lifted only after employees were clear of the load in that an employee had his hand on a sling hook when the load was lifted, allowing him to receive a crushing type injury.
