29 CFR 1910.147(c)(6)(ii) The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. (a) In that an examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about January 28, 2014, at worksite 1320 N Alliance, Springfield Missouri . Employees in theChemical lab were exposed to caught in between/crushing amputation hazard from the Tinius Olsen weld testing machine Model H25ks. When not using the safety stops attached to theTinius Olsen weld testing machine Model H25ks. (b) Employees in the Chemical lab were exposed to a struck by hazard when allowed to bypass computer program that stops the Tinius Olsen weld testing machine Model H25ks. when button is released which restrains the operator from inadvertently reaching into the point of operation.
