29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    Plant #3 - On or about January 31, 2014, per the employer's energy control procedures employees working on the W50 Mill used "e-stops" (control circuit devices) located at the operator's panel as a means of lockout rather than the 480 VAC electrical disconnects. The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules and techniques for the placement of locks on the main electrical disconnects for the W50 Mill. Employee(s) were thereby exposed to the hazards associated with the inadvertent start-up of the carriage containing the cut-off saw.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  Plant #3 - On or about January 31, 2014, the employer had not documented through a written certification that periodic inspections had been performed on the lockout / tagout procedures in the company's energy control program. Employee(s) were thereby exposed to the hazards associated with the inadvertent start-up of the carriage containing the cut-off saw.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    Plant #3 - On or about January 31, 2014, authorized employees (mill personnel) were required to perform setups, changeovers and remove jams (steel tube) on the W50 Mill. Authorized employees (mill personnel) were required to lockout "only" at the e-stops (control circuit devices) located at the operator's panel rather than the 480 VAC electrical disconnects. The employer did not provide adequate training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  Employees were thereby exposed to the hazards associated with the inadvertent startup of the mill and the carriage containing the cut-off saw.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    Plant #3 - On or about January 31, 2014, an employee working in the cut-off saw area of the W50 Mill was required to enter the gated station of the cut-off saw to remove a jam. The employee was not required to affix a lock to the 480 VAC electrical disconnect for the cut-off saw prior to entering into the gated station.  The employee was thereby exposed to the hazards associated with the inadvertent start-up of the carriage containing the cut-off saw.
