29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about February 4, 2014, on top of a structure, an employee using a Hitachi pneumatic air nail gun was not wearing safety glasses, exposing himself to being hit by nails or flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. On or about February 4, 2014, on top of a structure, two employees performing framing operations were not protected from a 13-foot 4-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  a. On or about February 4, 2014, the 32-foot Louisville extension ladder used to access and egress the top of a structure, did not extend 3 feet above the eave, exposing employees to a 13-foot fall hazard.
