29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about January 31, 2014, on the east side of the commercial construction site, employees were using extension cords that had the outer sheathing pulled away from the strain relief devices exposing internal wiring. The employees were exposed to electrical and shock hazards. a. Extension cord with the sheathing pulled away utilized to energize an Ostitch drill, b. Extension cord with the sheathing pulled away utilized to energize a Makita skill saw.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about January 31, 2014, on the east side of the construction site, an employee working from a basket elevated by a 8042 JLG sky track while engaged in decking activities on a commercial building was exposed to a fall hazard of approximately 19 feet.
