29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    In the Welding Shop, an employee was exposed to an amputation hazard from the six inch unused saw blade while operating a Wilton horizontal bandsaw.

29 CFR 1910.217(c)(2)(iv): An interlocked press barrier guard shall be attached to the press frame or bolster and shall be interlocked with the press clutch control so that the clutch cannot be activated unless the guard itself, or the hinged or movable sections of the guard are in position to conform to the requirements of Table O-10.    In the Tip Department, on or about November 23, 2013, an employee was exposed to an amputation hazard from the point of operation swing guard on the Bateman full revolution power press #34 which was not interlocked with the clutch control so that the clutch cannot be activated unless the guard conforms to Table O-10.

29 CFR 1910.217(e)(1)(i): It shall be the responsibility of the employer to establish and follow a program of periodic and regular inspections of his power presses to ensure that all their parts, auxiliary equipment, and safeguards are in a safe operating condition and adjustment. The employer shall maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected.    The Company had not developed nor implemented a program of periodic and regular inspections of the part and full revolution mechanical power presses and had not maintained a certification record of these inspections.

It shall be the responsibility of the employer to insure the original and continuing competence of personnel caring for, inspecting, and maintaining power presses.    The Company did not ensure that the maintenance employee had received the necessary training and instruction to properly inspect and maintain the part and full revolution power presses located in the facility.

29 CFR 1910.217(f)(2): Instruction to operators. The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall insure by adequate supervision that correct operating procedures are being followed.    On or about November 23, 2013, in the Tip Department, employees were was operating mechanical power presses such as but not limited to a Bateman full revolution press #34 and had not been adequately trained and instructed in the safe work method before starting work.

29 CFR 1910.217(b)(4)(ii): A pad with a nonslip contact area shall be firmly attached to the pedal.  In the Tip Department on the Batemen full revolution power press #34, the nonslip surface on the foot pedal was worn away from the pedal.
