29 CFR 1926.1053(b)(7): Ladder that was unsecured or not provided with slip-resistant feet was used on slippery surfaces: a) On or about 2/1/2014, at the above addressed jobsite, employees used the base section of an extension ladder that was not secured to prevent displacement and was placed on a slippery surface to access a roof, exposing the workers to a fall hazard. b) On or about 2/3/2014, at the above addressed jobsite, an employee used the fly section of an extension ladder on a slippery surface whereas the base of the ladder did not have slip-resistant safety shoes and was not secured to prevent displacement, exposing the employee to a fall hazard. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
