29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees a hazard created by rotating parts:           a) On or about January 27, 2014, at the above located work place, employees were assigned to work with a Hollymatic, 4200F Mixer Grinder. The rotating mixing paddle was not guarded, thereby exposing employees to laceration and amputation injuries.

29 CFR 1904.29(b) (l): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:      a) On or about February 6, 2014, at the above located workplace, the employer's OSHA 300 forms for 2013 was not completed in detail as required by 1904.1 through 1904.12. In that:      i) The OSHA 300 list of recordable injuries and illnesses included a days away from work injury that occurred on November 21, 2013. The entry had a check in column I (injury with transfer or restriction) and did not have a check in column H (injury with days away).

29 CFR 1910.333(b)(2)(i):   The employer did not maintain a copy of the written procedures outlined in 29 CFR 1910.333(b)(2) and did not make it available for inspection by employees:       a) On or about February 6, 2014, employees were required to work with premises wiring, lighting, machinery and other equipment. The employer did not have in place and/or in writing a safety-related work practices program as required by 29 CFR 1910.333(b)(2).   The employees were thereby exposed to electrical hazards.
