29 CFR 1918.55(c)(1): Accessible areas within the swing radius of the body of a revolving crane or within the travel of a shipboard gantry crane were not physically guarded to prevent an employee from being caught between the body of the crane and any fixed structure, or between parts of the crane.  Verbal warnings to employees to avoid the dangerous area do not meet this requirement.   M/V Westwood Columbia crane 1:  Westwood employee was allowed to walk between the vessel and the vacuum head.  This exposed him to a struck-by hazard where he was thrown into the side of the vessel and fell approximately 20 feet to the water.
