29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.    a)  Reynaldo Mancinas Stucco at 488 Spoon Court in Grand Junction, CO 81507 :  On or before January 29, 2014 the exposing/creating employer in charge of erecting the scaffolding at the worksite did not ensure that all supporting poles had a firm foundation, such as base plates and/or mud sills.  This condition exposed employees to fall and struck-by hazards.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports.     a)  Reynaldo Mancinas Stucco at 488 Spoon Court in Grand Junction, CO 81507 : On or before January 29, 2014 the exposing/creating employer did not ensure the scaffolding used to perform stucco work was fully planked so as to prevent employees from stepping through or falling from the scaffold.  This condition exposed employees to fall hazards.

29 CFR 1926.451(b)(3): Except as provided in paragraphs (b)(3)(i) and (ii) of this section, the front edge of all platforms shall not be more than 14 inches (36 cm) from the face of the work, unless guardrail systems are erected along the front edge and/or personal fall arrest systems are used in accordance with paragraph (g) of this section to protect employees from falling.  a)  Reynaldo Mancinas Stucco at 488 Spoon Court in Grand Junction, CO 81507 : On or before January 29, 2014 the exposing/creating employer did not ensure the front edge of the working platform was no more than 14 inches from the face of the work in that measurements indicated the front edge was at least 16 inches from the face of the work.  This condition exposed employees to fall hazards to the inside of the scaffold.

29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person:      a) Reynoldo Mancinas Stucco at 488 Spoon Court in Grand Junction, CO 81507: On or before January 29, 2014 the exposing/creating employer did not ensure the scaffold used to conduct stucco activities was was erected and/or altered under the supervision of a competent person.  Several deficiencies were observed with the scaffold such as but not limited to incomplete planking, missing mud sills, improper distance from the working face.  This condition exposed employees to fall and struck-by hazards.
