29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) Construction Enterprises, Inc., at 1308 W. Plum St., Fort Collins, CO 80521: On or before January 28, 2014, the exposing employer did not ensure that employees were protected from injury, in that an employee was elevated above the street from the basket of a JLG model 600S aerial lift and was not wearing fall protection. This hazard exposed the employee to a fall of approximately 20 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) Construction Enterprises, Inc., at 1308 W. Plum St., Fort Collins, CO 80521: On or before January 28, 2014, the exposing employer did not ensure that employees were protected from injury, in that two laborers were engaged in snow removal activities on an unfinished 5th floor of an apartment building under construction that had exposed edges and sides. This hazard exposed the employees to a fall of approximately 43 feet.
