29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):   Facility:  On, or about, February 10, 2014, a Hazard Assessment to determine what personal protective equipment was needed to protect employees had not been conducted.  Specific deficiencies were observed in the Laundry Department when changing containers of laundry chemicals and in the Housekeeping Department for floor stripping operations.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   Laundry:  On, or about, February 4, 2013, employees who changed five gallon containers of Commercial HD Detergent and Commercial White, both corrosive liquids, were not provided with adequate eye protection.  The safety glasses with side shield provided could not be worn over prescription glasses and the Safety Data Sheet for Commercial White indicated that chemical splash goggles should be worn.

The Plan did not address the post vaccination titer to be offered following the completion of the Hepatitis B vaccination series and did not address how non-responders to the Hepatitis B vaccination series should be handled.

Public Health Service current at the time these evaluations and procedure took place:  Nursing Units:  On, or about, February 10, 2014, employees had not been provided the Hepatitis B vaccination program in accordance with current CDC (Centers for Disease Control) and U.S. Public Health Guidelines in that:  A.      Two employees who worked as Certified Nursing Assistants had not been offered the post vaccination surface anti-body test to determine if the vaccination series had been effective.   B.     An employee who was a non-responder to the Hepatitis B vaccination series was not offered a medical evaluation.
