29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.       Workers were engaged in commercial construction activities disassembling an existing structure. Workers were exposed to potential between 10.6 and 12.4 fall hazards in that a proper fall protection system was not utilized.

29 CFR 1926.850(a): Evidence in writing was not available that an engineering survey performed by a competent person had been performed prior to permitting employee(s) to start demolition operations:(a) Workers were disassembling an existing structure (restaurant) located at 1065 Springfield St., Dayton , Ohio         Workers were engaged in commercial construction activities disassembling an existing structure. Workers were exposed to potential fall and struck-by hazards in that an engineering survey (demolition plan) by a competent person in writing was not available.
