29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:   Drill operating area. On or about March 27, 2014 and at time prior thereto, the drill operator was exposed to a continuous noise level dose of 218%, which converts to a time-weighted average (TWA) of 95.7 dBA.  Sampling was performed for 284 minutes during an 8 hour shift on March 27, 2014. Zero exposure was assumed for the time not sampled (196 minutes).  The employer had not established a hearing conservation program as described in 29 CFR 1910.95 (c) though (o) where employees were exposed to noise levels above the allowed 85 dBA TWA.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   Powder Coating production area. On or about February 4, 2014, and at times prior thereto, employees were exposed to corrosive materials, including but not limited to, Sodium Hydroxide and Phosphoric Acid, and were not provided with emergency eyewash and shower facilities for immediate use within the work area.  Employee(s) were exposed to potential chemical burns of the skin and eyes.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  Outside of plasma cutting warehouse. On or about February 4, 2014, and at times prior thereto, oxygen cylinders in storage were found to be stored within 1 foot of acetylene cylinders. Employee(s) were exposed to potential explosion and struck-by hazard(s).
