29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a) Employees were exposed to hazard of falling approximately 20 feet to the ground while performing carpentry work from elevated wood platform on the forks of a Lull 644B-42 all-terrain forklift that was not specifically designed by the manufacturer for lifting personnel. Gainer Park Carpentry, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(c)(2)(iv), which was contained in OSHA inspection number 497798, citation number 1, item number 1, and was affirmed as a final order on September 20, 2012, with respect to a workplace located at 407 E. Sunset, Arlington Heights, IL 60004. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
