29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about February 5, 2014, in the trusses, an employee using a pneumatic nail gun while installing framing components were not using eye protection, exposing himself to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.50(b):   a. On or about February 5, 2014, in the trusses, employees were installing transition boards while conducting framing operations at a height of approximately 26 feet above the ground without a fall arrest system, exposing themselves to a fall hazard.  b. On or about February 5, 2014, on the rear side of the house, an employees was installing framing components in order to support the trusses at a height of approximately 26 feet above the ground, exposing himself to a fall hazard.  c. On or about February 5, 2014, on the right side of the house, employees were installing framing components in order to support the trusses at a height of approximately 12 feet 9-inches above the ground, exposing themselves to a fall hazard.
