29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.     On or about January 30, 2014, Costco, Inc., did not document that the ventilation equipment, for the machinery room, complied with recognized and generally accepted good engineering practices such as, but not limited to,  IAPMO/ANSI UMC 1-2003 Uniform Mechanical Code.  Specifically, the ventilation was not run on a continuous basis during winter months.     This item was corrected during the inspection.

29 CFR 1910.119(j)(4)(i): The employer did not perform inspection and tests on process equipment     On or about January 30, 2014, Costco, Inc., did not perform the following inspection and tests on process equipment:     a.  An independent inspection was not conducted at least once every five years in accordance with recommended and generally accepted good engineering practices such as, but not limited to, IIAR Bulletin No. 110, Guidelines for: Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems, on the following pressure vessels: the High Pressure Receiver, the High Temperature Accumulator, the Medium Temperature Accumulator, and the Low Temperature Accumulator.     b.  Vibration Analysis was not conducted on Compressors # 1 - 9, as recommended by the manufacturer of the equipment, Johnson Controls, Inc.  The installation, operation, and maintenance manual specifies that vibration analysis should be conducted upon installation and every 6 months thereafter.     Abatement certification and documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
