29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On January 28, 2014, an employee was working underneath scaffolds with employees working on the scaffolds. The employee underneath was observed working without using any head protection from falling objects. This condition exposed the employee to struck by hazads from falling objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On January 28, 2014, an employee was working on a roof of a residential apartment complex under construction without using any form of fall protection. This condition exposed employees to falls greater than 6 feet to the ground.

29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm). On January 28, 2014 and at times prior thereto, employees were using a scaffold that was not fully planked. This conditon exposed employees to fall hazards greater than 10 feet.
