29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about January 24, 2014 and in times prior to and thereafter, employees were required to clean a Stimpson, Model 5822, meat grinder, located in the meat department without the employer developing and implementing a written Lockout/tagout program, exposing employees to the hazards of being caught in equipment that is not de-energized and locked out during servicing.

Workers grind hamburger meat and clean out the inside of the machine exposing them to the hazards of being caught in the grinder.

The employer had not developed a written hazard communication for protecting employees using chemicals, exposing employees to the hazards associated with chemicals.
