29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about February 5, 2014, at a work place located at 2211 S Southeast Loop 323, Tyler, TX 75701; at least one employee was working from the workman's basket of a Genie, Model Z-30/20N, (approximately 12 feet above the ground) without wearing a body belt or harness and a lanyard attached to the boom or basket.
