29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. On or about February 3, 2014, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2014: a. Laborer injured January 15, 2014, in the Saw Room on the Resaw when the employee fractured a left hand finger while feeding a board. b. Laborer suffered a low back strain while working in the Saw Room on January 23, 2014.
