OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the insulation of the electrical connections for an electromagnet lifting device was torn and in need of repair:    (a)  On or about January 27, 2014, and at times prior, the employer did not protect employees from falling and flying scrap metal from the magnet attached to a hook in the middle of the grapple on the Liebherr 904 material handler, which had the insulation of its electrical cord torn ripped and torn exposing the conductors inside.  The magnet was used to clean the area of scrap metal in close proximity to workers on the ground and operating other machinery during the clean-up activity.  Were the electrical connection to fail during a lift, the material would fall or be flung off of the magnet.
