29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a. On or about February 5, 2014, on the south side of the building, employees working on a scaffold platform did not have a guardrail system along all open sides and ends of the platform, exposing employees to a 25 feet fall hazard.
