29 CFR 1926.102(a)(1):   Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   (a) On or about January 30, 2014 at worksite located at 19301 E 50 Terr, Independence, Missouri. Employee was observed using a nail gun without any means of eye protection.

29 CFR 1926.404(f)(6):   The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  (a))On January 30, 2014, at a worksite located at 19301 E 50 Terr Independence Missouri, employees were exposed to serious electrical injuries, extension cords operating eletric saws and drills did not have a ground pin to ensure the path to ground was permanent, continuous and effective.

29 CFR 1926.501(b)(4):  Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  (a) ) On or about January 30, 2014, at worksite 19301 E 50 Terr Independence Missouri. Framers were working on the second story flat surface framing and building out the second floor with multiple holes that are uncovered and was exposed to an 17 foot fall to concrete.
