29 CFR 1926.300(b)(1): When power operated tools are designed to accommodate guards, they were not equipped with such guards when in use:    location: renovation area  on or about: 1/31/14    a) Employees, engaged in renovation work, were using a Ridgid Compound Miter Saw, model R412, to cut wood. The point of operation guard was missing, exposing the metal blade.

29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. The path to ground from circuits, equipment, and enclosures was not permanent and continuous:    location: renovation area  on or about: 01/31/14    a) Employees were using an extension cord to power electrical equipment, including a Ridgid abrasive cut-off machine. The extension cord was missing a grounding pin.

Hard service flexible cords No. 12 or larger were not repaired so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced:    location: renovation area  on or about: 01/31/14    a) Employees were using a spliced extension cord to power electrical equipment, including a Ridgid abrasive cut-off machine. The cord used for the splice did not have a grounding wire or pin and did not have the outer sheath properties of the original cord. Additionally, there was approximately two inches of the inner wires exposed.

Conductors entering boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors enter were not effectively closed. Unused openings in cabinets, boxes, and fittings also were not effectively closed:    location: renovation area  on or about: 1/31/14    a) Employees were using a duplex outlet that was no longer attached to metal conduit to energize electrical equipment, including a Ridgid abrasive cut-off machine. The wires going from the conduit to the duplex outlet were not protected from abrasion.

The ladder was not secured nor was the area barricaded.
