29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 dBA:  A hearing conservation program was not fully implemented where workers stationed at the pan oiler and ovens lines during dough stretching and de-panning were exposed to noise in excess of the of the 8-hour TWA of action level of 85 dba during their work shift. Audiometric and noise training programs were not implemented.   On March 21,  2014  a) Worker 1 was exposed to an 8 hour time weighted average of 92.9 dba (150% equivalent dose) while operating the pan oiler.  b) Worker 2 was exposed to an 8 hour time weighted average of 90.6 dba (109% equivalent dose) while at the oven line, stretching dough.   c) Worker 3 was exposed to an 8 hour time weighted average of 91.4 dba (121.7% equivalent dose) whille at oven line de-panning and stretching dough.   d) Worker 4 was exposed to an 8 hour time weighted average of 91.4 dba (122.2% equivalent dose) while at the oven line stretching dough.  e) Worker 5 was exposed to an 8 hour time weighted average of 92.9 dba (149.6% equivalent dose) while at pan oiler and oven line de-panning and stetching dough.  A hearing conservatrion program shall include:   1. Monitoring 2. Baseline and annual audiometric testing  3. Noise training 4. Posting of the hearing conservation standard.
