29 CFR 1926.852(e): Chute openings, into which workmen dumped debris, were not protected by substantial guardrails approximately forty-two inches above the floor or other surface on which men stood to dump material: Employees are exposed to falling over 40 feet to the ground below when dumping debris into the chute due to the lack of a guardrail to prevent employees from fall hazards.

29 CFR 1926.852(f): Where the material is dumped from mechanical equipment or wheelbarrows, a securely attached toeboard or bumper, not less than 4 inches thick and 6 inches high, was not provided at each chute opening: Employees are exposed to falling over 40 feet to the ground below when dumping debris into the chute via skid steer and wheelbarrow and a toeboard was not installed to prevent employees from fall hazards.
